Chibuokem Amuneke-Nze, PharmD, MBA
Chib Amuneke-Nze is an Inpatient Pharmacy Assistant Manager at The University of Texas MD Anderson Cancer Center in Houston, TX. He received his Bachelor of Science in Chemistry and Molecular Genetics from The Ohio State University and his Doctor of Pharmacy degree from The University of Texas College of Pharmacy. Chib was previously involved in ensuring optimal therapeutic appropriateness, safety and accuracy of ordered TPN, pediatric, and chemotherapy drug regimens for MD Anderson. He is currently involved with inpatient retrospective & prospective operations improvement programs examining the safety and quality of medication use processes via EHR analysis. He is also a member of The University of Texas at Austin College of Pharmacy Alumni Association and the Board of Directors for Building A Purpose which assists underprivileged students with academic mentorship. His professional interests include oncology informatics, pharmacy administration, transition of care, patient safety and medication use stewardship process improvement. |

Niaz Deyhim, PharmD, MS, BCPS
Dr. Niaz Deyhim received a Bachelor of Science in Biomedical Sciences at Texas A&M University – Kingsville, followed by a Doctor of Pharmacy at Texas A&M University Rangel College of Pharmacy. Dr. Deyhim is currently a Pharmacy Administrative Specialist at Houston Methodist Hospital and serves as the Pharmacist in Charge over Houston Methodist Continuing Care Hospital - Texas Medical Center. She also serves as Vice-President on the Texas A&M University Rangel College of Pharmacy Alumni Association Board of Directors. She completed a two-year Health-System Pharmacy Administration and Leadership residency program at Houston Methodist Hospital with concurrent pursuit of a Master of Science in Pharmacy Leadership and Administration at the University of Houston College of Pharmacy. The requirements that Dr. Deyhim fulfilled during her residency program and graduate degree curriculum solidified her clinical, operations, and administrative foundations, thereby providing the tools to advance pharmacy practice. Her long-term goals are to create meaningful and enduring pharmacy services that improve patient care and inspire future generations of pharmacists. Her professional interests include inpatient operations, research, regulatory compliance, academia, and medication safety. |

Reba Forbess, PharmD, PhD
Dr. Forbess graduated with her Doctor of Pharmacy Degree from The University of Tennessee Health Science Center in Memphis, TN. She went on to complete her Doctor of Philosophy Degree in Pharmaceutics, also from The University of Tennessee Health Science Center. In 2010, Dr. Forbess moved to Houston and began working at Houston Methodist Willowbrook. She established an Admission Medication Reconciliation Program and has an active interest in Medication Reconciliation, Transitions of Care and Patient Assistance. Dr. Forbess also works as a Clinical Pharmacist at HCA Houston Healthcare Kingwood. She currently serves as the TSHP Chair of the Membership Council. |

Christopher Murray, PharmD, MS, is the Regional Director of Pharmacy Operations for CommonSpirit Health South Region. Christopher received his PharmD from Hampton University in 2008, where he also received a minor in Leadership Studies. He then went on to complete his Health System Pharmacy Administration residency at Duke University Hospital while obtaining in parallel an MS in Health System Pharmacy from the University of North Carolina. Noor Zaidan, PharmD, BCIDP
Noor Zaidan is a Pharmacy Clinical Practice Specialist in Infectious Diseases at UTMB Health. Noor graduated with a Bachelor of Science in Human Nutrition and foods at the University of Houston in 2015 and received her Doctor of Pharmacy degree of the University of Houston College of Pharmacy in 2019. She went on to complete a PGY1 pharmacy residency and PGY2 residency in Infectious Diseases at UTMB Health in Galveston, TX. Her current practice involves rounding with the Infectious Diseases consult service and assists with implementing antimicrobial stewardship programs at her institution. Her current professional interests include antimicrobial stewardship, HIV, and global health. As someone who is from the Houston area, Noor is invested in contributing to the growth and development of her profession and community and now currently serves as the residency program coordinator for the PGY2 Infectious diseases pharmacy residency program at UTMB Health. |

Mohammad Qasim Adil, PharmD, BCPS
Dr. Mohammad Qasim Adil is a Clinical Pharmacy Specialist in Internal Medicine at Lyndon B. Johnson Hospital with Harris Health. Dr. Adil did his undergraduate coursework in Information Systems and pre-pharmacy at The University of Texas at Austin and went on to graduate from UT with his Doctor of Pharmacy degree in 2018. After graduation, he completed a PGY-1 Pharmacy Practice Residency at the Michael E. DeBakey VA Hospital in Houston, Texas. Dr. Adil has been extensively involved with SSHP as a student and scholarship recipient, AASHP as a co-chair of web communications and with TSHP as a member of the Educational Affairs, Clinical Practice and Residency Excellence committees. His professional interests include treatment of chronic cardiovascular diseases and diabetes, academia, preceptorship and pharmacy informatics. |

Divya Srinivasan, PhD, RPh
Divya Srinivasan is a pharmacy manager with a decade experience as a pharmacist. She graduated with a doctorate in Pharmacology at the University of Kentucky, Lexington, with a 4.0 GPA. Her PhD research focused on the use of Gleevac (a kinase inhibitor) in the treatment of breast cancer. Her work resulted in several publications and her article in “Cancer Research” was featured in the magazine cover. As a pharmacist, she is thrilled to have the opportunity to improve the quality of life of her patients. She is fortunate to have had the opportunity to establish an Antimicrobial Stewardship Program at St. Luke’s Brazosport and serve as the co-chair since 2016. During her tenure in this role, she created a multidisciplinary team to optimize antibiotic utilization. Divya started her leadership role as Manager in the thick of the COVID pandemic in May 2020. She is honored to work with an outstanding team at St. Luke’s which is driven to provide excellent, patient-centered care. Her biggest joy is in improving patient outcomes and serving her community. Her goal is continuous improvement and delivering exceptional patient care. |

Rochelle Akpaffiong, PharmD, MBA – HCM, DPLA
Rochelle Akpaffiong is an Inpatient Pharmacy Manager at The University of Texas Medical Branch, Clear Lake Hospital. She received her Doctor of Pharmacy degree from Texas Southern University College of Pharmacy and Health Sciences in 2008. Rochelle went on to obtain her MBA with a concentration in Health Care Management from The University of Texas at Tyler in 2016. In 2023 Rochelle obtained the designation of Diplomate, Pharmacy Leadership Academy (DPLA) after graduating from the American Society of Health-System Pharmacists Pharmacy Leadership Academy program. She is a member of several professional organizations, including American Society of Health-System Pharmacists, Texas Society of Health-System Pharmacists, the Gulf Coast Society of Health-System Pharmacists, and Lambda Kappa Sigma. Her professional interests include pharmacy leadership, mentorship, sterile compounding, medication safety, and drug diversion. |
Faith Rutherford, PharmD
Faith Rutherford is currently a PRN clinical pharmacist at Houston Methodist Willowbrook and HCA Houston Healthcare Kingwood. She received her PharmD from Texas Southern University College of Pharmacy and Health Sciences in 2023, followed by a PGY1 residency at Houston Methodist Willowbrook. Before and during pharmacy school, Faith worked as a hospital pharmacy technician for over 20 years and served in various specialized technician roles, such as pharmacy buyer and automation administrator. Faith joined GCSHP in 2015 and has been serving in various capacities ever since. Throughout her time with the organization, she has held roles as Technician Chair, Membership Engagement Committee member, Membership Committee member, and now as Membership Co-Chair. Faith also received the 2017 Mike Knapp Pharmacy Technician Award and the 2024 Leo F. and Ann Godley Residency Fellowship Award. Faith is a self-described cheerleader for the organization, believing that as members, they are able to make a significant impact on the pharmacy profession. |

Leslie Joseph, ILM
Leslie Joseph is an instructional lab manager (ILM) and has been at the University of Houston College of Pharmacy since 2021. In her role as an ILM, she assists faculty in the clinical skills lab setting with logistics and setting up lab beforehand. She also works directly with Pharm.D. students via select teaching and evaluation responsibilities. Specializing in IV validation and having ten years of hospital experience, Leslie uses her experience to help students in learning vital parts of inpatient pharmacy practice skills. Her goal is to ensure that students have a better understanding of how the world after pharmacy school looks. Leslie has been able to put her Bachelor of Science degree in Health Sciences earned from Grand Canyon University to effective use. Over the years, Leslie’s strengths at Memorial Hermann at the Texas Medical Center have garnered some recognition for IV validations. The Pharm.D. students' welcoming smiles keep her going and inspire Leslie in her work. When she’s not at University of Houston College of Pharmacy, Leslie is an avid foodie and loves spending time traveling with her family. She also volunteers for student functions at UH. |
Sherrie M. Myers, RCPhT
Sherrie currently occupies a position as an Instructional Lab Manager at the University of Houston College of Pharmacy, where she performs hands on instructional skills labs with first to third year Pharm.D. students in addition to assisting faculty with logistics, course content and skills lab management. Sherrie also currently holds a part time position with Hospital Corporation of America (HCA) Houston Northwest and a shared employee position at HCA Houston Kingwood where she has been an employee since 2016. Sherrie has experience as a Pharmacy Technician since 1990. She began in the retail setting then moved into hospital pharmacy in 1998. Prior to her time with University of Houston and HCA, Sherrie worked at University of Maryland Medical Center in Baltimore, Maryland where she held several leadership positions during her fifteen years there. These leadership positions included Investigational Drug Service Specialist, Lead Technician of the Shock Trauma Pharmacy, Gudelsky Pharmacy, Central IV Room and Training and Staff Development Coordinator. Her specialty is training and staff development with emphasis on sterile products compounding and IV validation. In her free time, Sherrie enjoys cooking, spending time with her two adult children, volunteering for student functions at UH and relaxing on the beach. |

Valerie Buckley, PharmD, BCIDP
Valerie is the Infectious Diseases Clinical Pharmacy Specialist at Memorial Hermann Southeast Hospital. She completed her PGY2 Infectious Diseases pharmacy residency at the University of Texas Medical Branch and her PGY1 at Memorial Hermann Memorial City Medical Center in Houston. She is originally from Las Vegas, Nevada and graduated from Roseman University of Health Sciences with a PharmD and University of Nevada, Las Vegas with a bachelor's in biology. Valerie plans to pursue board certification in Infectious Diseases and would like to explore opportunities for teaching and precepting in her current role. Her areas of interest within ID include antimicrobial resistance and oral antibiotics for severe infections. |

His primary roles are in Buy & Bill, Contracting, Partnering with Pharmacy Directors and launching new oncology therapies for Breast, Gastric, Lung, and AML. Rick has been a member of TSHP for 7 years and has served as Industry Chair for the Gulf Coast Chapter for the past 5. Rick earned his Bachelor of Science from Elon University in North Carolina. |

Emily Hobaugh, MS serves as the GCSHP Industry Co-Chair and TSHP Industry Section Chair. She is a Neuroscience District Business Manager at Takeda Pharmaceuticals, Inc. Emily has held multiple roles working across Rare Disease, Oncology, Hematology, GI, and Neuroscience in her 23+ year career at Takeda. She also serves as the US Development & Mentoring Lead for Takeda’s Employee Resource Group. Emily brings over ten years of non-profit leadership experience and volunteers for multiple non-profit organizations in Houston. She was recognized by the Houston Business Journal in 2020 for her leadership launching a non-profit professional development association in Houston to further advance the impact of women in the business of healthcare. Ashley Crouch, PharmD
Ashley graduated from The University of Texas at Austin College of Pharmacy (UTCOP) with a Doctor of Pharmacy degree in 2019. She went on to complete an ASHP accredited PGY-1 Pharmacy residency at Massachusetts General Hospital and PGY-2 Internal Medicine residency at Ascension Seton. In August 2021, she joined The University of Texas MD Anderson Cancer Center as a Clinical Pharmacy Specialist of Hospital Medicine. In this role she works inpatient to optimize the pharmacotherapy of solid tumor patients admitted for complications of cancer and cancer treatment. Outside of clinical practice, Ashley serves as the Regional Health-System Coordinator for UTCOP in the Galveston/Houston Region where she facilitates experiential learning experiences for fourth year pharmacy students. I am the current SSHP President for the 2024-2025 school year as well as Class President for the Class of 2026. After completing pharmacy school, I hope to apply for residency but I am not sure yet of what to specialize in. My current interests include ambulatory care, pediatrics, and managed care. William Pham
William Pham is a 3rd-year professional student pharmacist at Texas A&M Irma Lerma Rangel School of Pharmacy. William Pham, born and raised in El Paso, Texas, completed his pharmaceutical prerequisite courses during his undergraduate years at Texas A&M University. William Pham’s first two years at the School of Pharmacy consisted of serving multiple leadership roles such as the Advocacy Co-Chair and Vice President Elect for SSHP, Fundraising Chair for APhA, and Chaplain for Kappa Psi Pharmaceutical Fraternity. Now as Vice President Elect for the 2024-2025 academic year, he will be continuing to serve SSHP by helping the organization grow, serve members and the community, and share the mission of ASHP and TSHP. Currently, Mr. Pham hopes to one day pursue a future in Ambulatory Care at a Hospital or work in the VA with the Navy to help one day give back to his community. |
